Transcript Request

Mrs. Amecia Paley, Registrar - Office Location - "MHS Main Front Office"

The student visit the registrar's office and request a form to complete and return as soon as possible. There is a box for blank forms and one for completed forms. Once the request form has been completed, the student will then place the form in the box labeled completed. The registrar will then complete the request. Most of the time the transcripts are sent by mail unless the school gives the student an email to send it electronically.
